Folate Antagonists, Fluoroquinolones & Metronidazole

Drugs that starve bacteria of folate or break their DNA — and the urinary antiseptic nitrofurantoin.

FOCUS ON
Sulfonamides (sulfamethoxazole) block dihydropteroate synthase (PABA analog); trimethoprim blocks bacterial dihydrofolate reductase → together (TMP-SMX) bactericidal: UTI, Pneumocystis (treatment + prophylaxis), Toxoplasma prophylaxis, Nocardia, MRSA skin, Shigella. Toxicity: hypersensitivity/SJS, hemolysis in G6PD deficiency, hyperkalemia (TMP blocks ENaC like amiloride), ↑ creatinine, kernicterus in newborns, megaloblastic anemia, warfarin potentiation. Fluoroquinolones (cipro, levo, moxi) inhibit DNA gyrase (topo II) and topo IV: UTI, pneumonia (levo, moxi), Pseudomonas (cipro, levo), anthrax; toxicity: tendon rupture, QT prolongation, cartilage damage (children), CNS effects, aortic aneurysm, dysglycemia. Metronidazole: free radicals damage DNA (anaerobes, protozoa) → disulfiram reaction, metallic taste.

MECHANISM
USES
TOXICITY · NOTES
Sulfonamides (SMX, sulfadiazine)
PABA analog → inhibit dihydropteroate synthase
Sulfadiazine + pyrimethamine for Toxoplasma; silver sulfadiazine for burns
Hypersensitivity, SJS/TEN, photosensitivity, G6PD hemolysis, crystalluria, kernicterus, interstitial nephritis
Trimethoprim
Inhibits bacterial dihydrofolate reductase
Combined with SMX
Hyperkalemia (ENaC block), ↑ creatinine (blocks secretion), megaloblastic anemia, leukopenia (give leucovorin)
TMP-SMX uses
Sequential blockade → cidal
Cystitis, Pneumocystis, Toxoplasma prophylaxis, Nocardia, MRSA skin, Shigella, Listeria (alternative), Cyclospora, Stenotrophomonas
Potentiates warfarin, phenytoin (CYP2C9)
Ciprofloxacin
Inhibits DNA gyrase (gram ⁻) and topoisomerase IV (gram ⁺) → DNA breaks; cidal
UTI, pyelonephritis, Pseudomonas, traveler's diarrhea, anthrax, meningococcal prophylaxis
Weak gram ⁺ coverage
Levofloxacin · moxifloxacin
"Respiratory" FQs; better gram ⁺ and atypicals
Community pneumonia, MDR TB (moxi, levo); moxi covers anaerobes
Moxi poor urine levels
FQ toxicity
Collagen and cartilage effects; hERG block
Tendinitis/Achilles rupture (age > 60, steroids, transplant), QT prolongation, aortic aneurysm/dissection, CNS (seizures, delirium), peripheral neuropathy, dysglycemia, C difficile
Avoid in children and pregnancy (cartilage) · chelated by antacids, iron, calcium
FQ resistance
Mutations in gyrase (gyrA) and topo IV; efflux
Gonorrhea now resistant
—
Metronidazole
Reduced in anaerobes → free radicals damage DNA
Anaerobes below the diaphragm, C difficile (mild, alternative), BV, Trichomonas, Giardia, Entamoeba, H pylori
Disulfiram-like reaction with alcohol, metallic taste, peripheral neuropathy
Nitrofurantoin
Reactive intermediates damage DNA, ribosomes; concentrates in urine
Uncomplicated cystitis, prophylaxis; safe in most of pregnancy
Pulmonary fibrosis/pneumonitis, hemolysis in G6PD, peripheral neuropathy · avoid in pyelonephritis and low GFR
Rifamycins
Inhibit DNA-dependent RNA polymerase
TB, meningococcal/Hib prophylaxis, staph biofilm adjunct

Human vs bacterial targets
Humans take up folate (no dihydropteroate synthase); bacterial DHFR differs from human
Selective toxicity
Methotrexate hits human DHFR; pyrimethamine hits protozoal

Mechanism — sequential folate blockade

DHFR⊣ trimethoprim, pyrimethamine

How it's tested

COMMON CONFUSIONHyperkalemia on TMP-SMX (amiloride-like ENaC block) vs true AKI — trimethoprim also raises creatinine by blocking tubular secretion without lowering GFR.
THE TRAPFluoroquinolones and tetracyclines lose absorption with antacids, calcium, iron, zinc — separate the doses.
YOUR APPROACHTarget (folate, gyrase, DNA radicals) → spectrum niche → host risk (G6PD, elderly tendons, pregnancy, newborns, QT) → interactions.

If you see… think…

Sulfonamide target → dihydropteroate synthase
Trimethoprim target → bacterial DHFR
Potassium 6.1 in an HIV patient on high-dose TMP-SMX → ENaC blockade
Blistering mucosal rash after a sulfa drug → SJS/TEN
Elderly patient on prednisone with heel snap on levofloxacin → Achilles rupture
Fluoroquinolone target → DNA gyrase / topoisomerase IV
Flushing and vomiting after wine on BV treatment → metronidazole disulfiram reaction
Cough and lung fibrosis on long-term UTI prophylaxis → nitrofurantoin
Newborn kernicterus risk from displacement of bilirubin → sulfonamides
Respiratory FQ with anaerobic coverage → moxifloxacin
