Herpesviruses I — HSV & VZV

Enveloped dsDNA viruses that hide in sensory ganglia for life and reactivate along nerves — vesicles on an erythematous base and multinucleated giant cells.

FOCUS ON
HSV-1: gingivostomatitis, herpes labialis, keratitis, temporal lobe encephalitis (most common sporadic viral encephalitis), herpetic whitlow; latent in trigeminal ganglia. HSV-2: genital herpes, neonatal herpes, aseptic meningitis; latent in sacral ganglia. VZV: chickenpox (lesions in different stages), latent in dorsal root/trigeminal ganglia → shingles in a dermatome; zoster ophthalmicus, Ramsay Hunt. Tzanck smear → multinucleated giant cells (PCR is the test of choice). Acyclovir/valacyclovir; live vaccine (varicella) and recombinant zoster vaccine.

CLINICAL PICTURE
CLUE · TREATMENT
Gingivostomatitis
Primary HSV-1 in children
Fever, painful oral vesicles and ulcers, drooling, poor feeding
Supportive ± acyclovir
Herpes labialis
HSV-1 reactivation (stress, UV, fever)
Tingling then grouped vesicles on the lip border
Valacyclovir early
HSV keratitis
HSV-1 in ophthalmic nerve
Painful red eye, photophobia, dendritic corneal ulcer on fluorescein
Avoid steroids · topical/oral antivirals
HSV encephalitis
HSV-1 spreads to temporal lobes
Fever, confusion, personality change, aphasia, seizures; hemorrhagic necrosis
CSF lymphocytes + RBCs, PCR · MRI temporal lesions · IV acyclovir immediately
Genital herpes
HSV-2 (increasingly HSV-1)
Painful grouped vesicles → ulcers, tender nodes, dysuria; recurrent milder
PCR of lesion · valacyclovir; suppressive therapy
Neonatal herpes
Passage through canal with active lesions
Skin-eye-mouth, CNS or disseminated disease in first weeks
C-section if active lesions · IV acyclovir
Herpetic whitlow
HSV inoculated into finger
Painful vesicles on fingertip (healthcare workers, thumb-sucking children)
Don't incise
Eczema herpeticum
HSV on atopic skin
Punched-out erosions over eczema patches
Acyclovir
Chickenpox (varicella)
Primary VZV via respiratory droplets
Fever, itchy "dewdrop on a rose petal" vesicles starting on trunk, crops in different stages
Contagious until all crusted · adults: pneumonia · avoid aspirin (Reye)
Shingles (zoster)
VZV reactivation in sensory ganglion (age, immunosuppression)
Burning pain then vesicles in one dermatome, not crossing midline
Valacyclovir within 72 h · recombinant zoster vaccine ≥ 50
Zoster ophthalmicus · Ramsay Hunt
V1 dermatome · geniculate ganglion (CN VII)
Hutchinson sign (vesicles on nose tip) → eye involvement · facial palsy + ear vesicles
Urgent ophthalmology
Postherpetic neuralgia
Nerve damage after zoster
Pain persisting > 3 months
Gabapentin, TCAs, lidocaine patch
Congenital varicella
Maternal infection in early pregnancy
Limb hypoplasia, cicatricial skin scars, eye defects, CNS damage
VZIG for exposed non-immune pregnant women

How it's tested

COMMON CONFUSIONVaricella shows lesions in every stage at once; smallpox lesions are all at the same stage and denser on face and limbs.
THE TRAPIn suspected HSV encephalitis, start IV acyclovir before the PCR result — delay worsens outcome.
YOUR APPROACHVesicles → distribution (oral, genital, dermatome, generalized) → virus → host risk → acyclovir route.

If you see… think…

Fever, confusion and temporal lobe lesion on MRI → HSV-1 encephalitis
Dendritic corneal ulcer → HSV keratitis
Multinucleated giant cells on Tzanck smear → HSV or VZV
Painful vesicles on a dental hygienist's finger → herpetic whitlow
Itchy vesicles in different stages starting on trunk → varicella
Band of painful vesicles that stops at the midline → herpes zoster
Vesicle on the tip of the nose with zoster on forehead → zoster ophthalmicus
Facial palsy with vesicles in the ear canal → Ramsay Hunt
Newborn with vesicles and seizures in week 2 → neonatal HSV
Drug for all of these → acyclovir / valacyclovir
