Membranes, Na⁺/K⁺-ATPase & Signaling

The pump that sets every gradient, and the receptor families that turn a hormone outside into an enzyme switched on inside.

FOCUS ON
Na⁺/K⁺-ATPase moves 3 Na⁺ out, 2 K⁺ in per ATP; digoxin blocks it → ↑ intracellular Ca²⁺ → stronger contraction. Receptors: Gs → ↑ cAMP → PKA; Gi → ↓ cAMP; Gq → PLC → IP₃ (Ca²⁺) + DAG (PKC). Receptor tyrosine kinases (insulin, growth factors) → MAPK/PI3K. JAK-STAT = cytokines, GH, prolactin, EPO. Steroids, thyroid hormone, vitamin D go to intracellular receptors.

EXAMPLES
CLUE · DRUG
Na⁺/K⁺-ATPase
3 Na⁺ out, 2 K⁺ in, 1 ATP; phosphorylated on the cytosolic side
All cells; drives secondary transport
Digoxin, ouabain bind the K⁺ site → inhibit
Gs
Adenylyl cyclase ↑ → cAMP → protein kinase A
β₁, β₂, D₁, H₂, V₂ receptors; glucagon, PTH, TSH, ACTH, LH/FSH, calcitonin, CRH, GHRH
Cholera toxin locks Gs on
Gi
Adenylyl cyclase ↓ → ↓ cAMP
α₂, M₂, D₂, opioid receptors, GABA-B
Pertussis toxin blocks Gi (↑ cAMP)
Gq
Phospholipase C → IP₃ (↑ Ca²⁺ from ER) + DAG (PKC)
α₁, M₁, M₃, H₁, V₁; GnRH, TRH, oxytocin, angiotensin II, gastrin
H₁, α₁, V₁, M₁, M₃ receptors
cGMP pathways
Guanylyl cyclase → cGMP → PKG → smooth muscle relaxation
ANP/BNP (receptor GC), nitric oxide (soluble GC)
Nitrates, sildenafil (blocks PDE5 breakdown)
Receptor tyrosine kinase
Ligand → dimerization → autophosphorylation → RAS–MAPK, PI3K–Akt
Insulin, IGF-1, EGF, PDGF, FGF, VEGF
Imatinib, erlotinib, trastuzumab (HER2)
JAK-STAT (non-receptor TK)
Receptor-associated JAK phosphorylates STAT → nucleus
Cytokines (IL-2, IL-6, interferons), GH, prolactin, EPO, G-CSF, thrombopoietin
JAK2 V617F (polycythemia vera) · tofacitinib, ruxolitinib
Intracellular receptors
Lipophilic hormones cross the membrane → zinc-finger receptor → DNA response element
Cortisol, aldosterone, sex steroids, T₃/T₄, vitamin D, retinoic acid
Slow onset, long-lasting effects
Ligand-gated ion channels
Neurotransmitter opens a channel directly (fast)
Nicotinic ACh, GABA-A, NMDA, 5-HT₃
Benzodiazepines, neuromuscular blockers, ondansetron
Membrane lipids
Phospholipid bilayer with cholesterol (↓ fluidity, ↑ stiffness), sphingolipids and glycolipids
Myelin rich in sphingomyelin
Spur cells from excess membrane cholesterol (liver disease)

Mechanism — why digoxin strengthens the heart

DigoxinBlocks Na⁺/K⁺-ATPase
→
↑ Intracellular Na⁺
→
Na⁺/Ca²⁺ exchanger slows
→
↑ Intracellular Ca²⁺
→
↑ ContractilityPlus vagal slowing of AV node

Hypokalemia frees the K⁺ site → more digoxin binding → toxicity (nausea, yellow vision, arrhythmias).

How it's tested

COMMON CONFUSIONInsulin uses a receptor tyrosine kinase; GH, prolactin and cytokines use JAK-STAT (the receptor itself is not a kinase).
THE TRAPPertussis toxin raises cAMP too — by disabling the inhibitory Gi, not by activating Gs.
YOUR APPROACHWater- or fat-soluble hormone? → membrane receptor family → messenger → kinase → toxin or drug acting there.

If you see… think…

Digoxin toxicity worsened by → hypokalemia
Rice-water diarrhea, toxin locks adenylyl cyclase on → cholera toxin (Gs)
Toxin disabling Gi → pertussis toxin
α₁, M₃, H₁, V₁ receptors → Gq → IP₃/DAG
Polycythemia vera mutation → JAK2 V617F
Insulin receptor type → receptor tyrosine kinase
Cortisol and T₃ receptors → intracellular (nuclear) receptors
Sildenafil effect → ↑ cGMP (PDE5 block)
GH, prolactin, EPO signal via → JAK-STAT
Pump moves 3 Na⁺ out, 2 K⁺ in → Na⁺/K⁺-ATPase
